On June 9, 2015 employees were using a 50 foot flexible cord on the jobsite which had the outer insulating jacket pulled out of the receptacle end; exposing the inner conductors.

On or about June 9, 2015 employees were using a 50 foot yellow flexible cord to energize a Skilsaw. The cord had a 1-2 inch section of the yellow outer insulation damaged/ removed exposing the inner conductors of the flexible cord.
