29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    On the jobsite located at 9133 Summit Street, Omaha, NE: The employer is failing to ensure that flexible cords were provided strain relief. On June 9, 2015 employees were using a 50 foot flexible cord on the jobsite which had the outer insulating jacket pulled out of the receptacle end; exposing the inner conductors.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:    On the jobsite located at 9133 Summit Street, Omaha, NE: The employer is failing to ensure that flexible cords that are frayed, worn and/or damaged were not used by employees. On or about June 9, 2015 employees were using a 50 foot yellow flexible cord to energize a Skilsaw. The cord had a 1-2 inch section of the yellow outer insulation damaged/ removed exposing the inner conductors of the flexible cord.

On or about June 9, 2015 employees were observed performing residential framing while exposed to fall distances of approximately 17 feet.

On the jobsite located at 9133 Summit Street, Omaha, NE: The employer is failing to protect employees from falls while working near wall openings where fall distances are greater than 6 feet to the next lower level. On or about June 9, 2015, an employee was observed working inside of the residence being framed, near a wall opening which was 11 feet above the ground.

On or about June 9, 2015, Employees of Ramirez Construction were observed performing residential framing construction and exposed to fall distances greater than 6 feet without adequate fall protection such as but not limited to guardrails, safety nets, personal fall arrest systems, etc.
