29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about June 4th, 2015: Employees engaged in the task of webbing the 360 S200 printing press (a Goss Sunday 2000 / 16 web offset printing press), were not required to: (1) Turn off and shut down the machine; (2) Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source; (3) Affix lockout or tagout devices to each energy isolating device; (4) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished. Employees that were required to rely on a method of placing the printing press in a continuous "slow run" or "crawl" mode and hand-feed the paper through each printing unit were exposed to an in-going nip point hazard from rotating press rollers located within the printing units. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
