29 CFR 1910.95(d)(1): When information indicates that any employee's exposure may equal or exceed an 8-hour time-weighted average of 85 decibels, the employer shall develop and implement a monitoring program. The employer did not develop and implement a monitoring program as required. On or about March 4, 2022, an employee was exposed to noise levels above an 8-hour time weighted average of 85 dBA. A packager was exposed to an 8-hour TWA of 88.1 dBA (Equivalent Dose of 71.1%). Time sampled was 450 minutes with a zero increment for 30 minutes which were not sampled. The employer did not develop and implement a monitoring program as required. This item was corrected during the inspection.

On or about March 4, 2022, an employee was exposed to noise levels above an 8-hour time weighted average of 85 dBA. A packager was exposed to an 8-hour TWA of 88.1 dBA (Equivalent Dose of 71.1%). 29 CFR 1910.95(k)(1):The employer shall train each employee who is exposed to noise at or above an 8-hour time weighted average of 85 decibels in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program. On or about March 4, 2022, an employee was exposed to noise levels above an 8-hour-time weighted average (TWA) of 85 dBA. The employee was not provided any training on noise and the hearing conservation program. The employer did not ensure that all employees participate in the training program. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.13(c).
