29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about December 2, 2016, the employer did not ensure employees were protected from the unexpected startup of components by performing the following sequence of actions prior to servicing, set up, and maintenance of the 360 S200 printing press (Goss Sunday 2000/16 web offset): 1) Machine or equipment shutdown 2) Machine or equipment isolation 3) Lockout or tagout device application 4) Verification of isolation Employees were exposed to hazards while engaged in the following work activities on the printing press: a) Employees were exposed to amputations when webbing the Goss Sunday 2000 printing press without using the inch-safe method. b) Employees were exposed to amputations while cleaning rollers on the Goss Sunday 2000 printing press. c) Employees were exposed to amputations, embolisms, and electrical hazards while changing blankets and setting/adjusting printing rolls in the panel of the Goss Sunday 2000 printing press. American Litho Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1069987 citation number 1, item number 1 and was affirmed as a final order on 12/17/2015 with respect to a workplace located at 175 Mercedes Dr. Carol Stream, IL 60188. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
