29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about June 9, 2015, at Lot 25A, 2732 Collins Cove Ave., Lawrenceville, Georgia:  One employee performing roofing work was exposed to struck-by hazard from falling shingles and nail guns when he worked beneath the employees above him without using head protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 9, 2015, at Lot 25A, 2732 Collins Cove Ave, Lawrenceville, Georgia:  Four employees performing roofing work were exposed to a fall hazard of approximately 10.3-feet when they used an aluminum extension ladder that did not extend three feet above the porch roof landing.

Brothers Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 989769, citation number 1, item number 1, and was affirmed as a final order on February 10, 2015, with respect to a workplace located at 6800 Story Circle, Norcross, Georgia, 30071.
