29 CFR 1910.147(c)(2)(ii): An energy isolating device that was capable of being locked out, did not utilize a lockout device, which rendered the machine or equipment inoperative: a) At the shavers, on June 2, 2015, the employer did not ensure the use of the company's lockout tagout program during the re-fitting of the drive belt of the feed conveyor, exposing an employee to amputation, debriding, and amputation injuries.
