Shaver Bin Area # 1, 3, and 4 - On or about and times prior to January 13, 2021, the employer did not ensure employees were protected from stepping into or through a gap between the elevated platform (catwalk) and a shaver bin, thus exposing employees trip and fall, stuck-by, and caught-in hazards.

29 CFR  1910.29(b)(1): The employer did not ensure that  guardrail systems met the requirement that the top edge height of top rails, or equivalent guardrail system members are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface.  a). Shaver Bin # 4 Area - On or about and times prior to January 13, 2021, the height of the top guardrail on a section of an elevated platform (catwalk) was approximately 36 inches, thus exposing employees to a fall hazard of approximately 8.5 feet.  b). Shaver Bin # 3 Area - On or about and times prior to January 13, 2021, the height of the top guardrail on a section of an elevated platform (catwalk) was approximately 24 inches, thus exposing employees to a fall hazard of approximately 8.5 feet.
