Rosenboom Machine & Tool, Inc. located at 1032 Maple St. Bowling Green, OH; On or about June 18, 2015, the employer did not ensure employees were protected while changing dies on the Alpine Pipe Bender. Employees are required to change dies on the bender and do not deenergize the equipment, exposing them to amputation hazards.
