29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: The employer is failing to protect employees and contractors from electrical shock and amputation hazards. The employers Lock-out/Tag-out program does not address or delineate which program will be used when the outside contractors are working in the facility. The current program does not address the requirements of this subpart.

29 CFR 1910.1200(h)(2)(iii): The employer did not provide information to the employees as to the location and availability of the written hazard communication program, and material safety data sheets required by 29 CFR 1910.1200: The employer is failing to provide adequate Hazard Communication training to employees either during their initial assignment or whenever a new physical or health hazard that the employee has not previously been trained for has been introduced into their work area. Some of the employees in the facility were not aware of the location or purpose of the Safety Data Sheets (SDS) in the facility.
