29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: The employer is failing to protect employees from laceration hazards by not properly guarding equipment. The work rest on the bench grinder in the maintenance shop was not adjusted adjacent to the wheel to the required 1/8" gap. The gap measured at approximately 3/8".

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: The employer is failing to ensure that the tongue guard of the bench grinder is properly adjusted to protect employees from laceration hazards. The tongue guard on the grinder in question was not correctly adjusted at the time of the inspection.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: The employer is failing to ensure that the areas in front of circuit breaker panel boxes are free from obstruction and able to be readily accessed. The panel box located in the Maintenance Building was blocked by numerous pieces of equipment and materials. This impedes access to the box in an emergency situation in which the electricity would need to be turned off at the box.
