29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: The employer is failing ensure that employees are protected from struck-by hazards. This most recently occurred at 1508 W 1st Street, Ogallala, Nebraska 69153 on August 9, 2017. Employees were using a Dewalt Bench Grinder which had the abrasive wheel periphery opening of 1/2 of an inch. Kelley Bean Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(b)(9), which was contained in OSHA inspection number 1109520, Citation number 1, Item number 1 and was affirmed as a final order on 01/08/2016, with respect to a workplace located at 1508 W 1st Street, Ogallala, Nebraska 69153.
