29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.    a)  Jobsite, 6th Street & Fairmount Ave., Philadelphia, PA - Guardrails were not installed on the open end of outrigger platforms, exposing employees to fall hazards of approximately twenty two (22) feet.  Observed on or about 6/9/15.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.    a)  Jobsite, 6th Street & Fairmount Ave., Philadelphia, PA -  Employees conducting brink laying operations were on   the ground, 3rd level and 4th level of the scaffold and were not protected from falling objects such as tools, materials, loose bricks, etc. Toeboards were not installed to prevent objects from falling to the lower level and exposing employees to struck by hazards.  Observed on/about 6/9/15.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a)  Jobsite, 6th Street & Fairmount Ave., Philadelphia, PA -  Employees were conducting brink laying operations on the 3rd and 4th level of fabricated scaffold and were not provided fall protection.  Employees were exposed to fall hazards of approximately 22 feet above ground level.  Observed on or about 6/9/2015.         Reiss Masonary Construction,  LLC was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.451(g)(1)(vii), which was contained in OSHA Inspection Number 981729, Citation #1, Item 1a issued on August 14, 2014 with respect to a workplace located at 142 Race Street, Philadelphia, PA 19106 and became final order on September 9, 2014.
