For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.    a)  Jobsite, 6th Street & Fairmount Ave., Philadelphia, PA -  Employees conducting brink laying operations were on   the ground, 3rd level and 4th level of the scaffold and were not protected from falling objects such as tools, materials, loose bricks, etc. Toeboards were not installed to prevent objects from falling to the lower level and exposing employees to struck by hazards.  Observed on/about 6/9/15.

Reiss Masonary Construction,  LLC was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.451(g)(1)(vii), which was contained in OSHA Inspection Number 981729, Citation #1, Item 1a issued on August 14, 2014 with respect to a workplace located at 142 Race Street, Philadelphia, PA 19106 and became final order on September 9, 2014.
