29 CFR 1926.300(a): Hand tool(s), power tool(s) or similar equipment were not maintained in a safe condition:   On or about June 24, 2015, at the above addressed site, an employee was exposed to a potential electrical shock and or burn while using an Dewalt screw gun that had the end of the power cord severed and replaced with a section of electrical extension cord with the internal wires exposed.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  On or about June 24, 2015, at the above addressed site, employees were exposed to a potential fall hazard while using an extension ladder set up on an active construction site to gain access to and from a roof while the ladder was not secured to prevent accidental displacement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about June 24, 2015, at the above addressed site, employees were exposed to a potential fall hazard while performing roof work installing tiles on the roof of a residence with unprotected sides and edges approximately 10 feet above a lower level while not being protected from falling to that lower level.  CARDINAL ROOFING & SIDING COMPANY, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVILANT STANDARD 29 CFR 1926.501(b)(11), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 948653, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON 03/24/2014, WITH RESPECT TO A WORKPLACE LOCATED AT 23472 SANABRIA LOOP, BONITA SPRINGS, FL.
