29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.   a)  On or about 06-25-15 at 401 N Washington St. in Green Bay, employer did not provide written certification of required fall protection safety training for employees working at heights greater than six (6) feet.  Four employees had been working on balconies at heights greater than 10, 20, and 30 ft. with two of the employees working on the top of awnings at greater than 10 ft. above surface level.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.    a)  On or about 06-25-15, employees were using lifelines wrapped around door/window jambs as the anchor points of personal fall arrest systems at heights greater than 10 feet above surface level.  The lifeline was not designed to be used as anchor equipment in a personal fall arrest system.
