29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Jobsite, 7th Street & Fairmount Ave., Philadelphia, PA - Employees performing framing operations, were exposed to fall hazards up to approximately thirty-one (31) feet, and were not protected by guardrail systems, safety net systems or personal fall arrest systems.  Observed on or about 6/12/15.  SIMPLE CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(13) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1042949, CITATION 1 ITEM 3, AND WAS AFFIRMED AS A FINAL ORDER ON 5/27/15 WITH RESPECT TO A WORKPLACE LOCATED AT 43 STAMFORD HILL ROAD, BLOCK 1159, LOT 19.25, LAKEWOOD NJ.    SIMPLE CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(13) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1046032, CITATION 1 ITEM 2, AND WAS AFFIRMED AS A FINAL ORDER ON 5/27/15 WITH RESPECT TO A WORKPLACE LOCATED AT 3 MARY LANE, BLOCK 26, LOT 2, ROBBINSVILLE NJ.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards:  a) 7th Street & Fairmount Ave., Philadelphia - Employees performing framing operations were not trained in the recognition, avoidance and mitigation of fall hazards.  Employees were exposed to falls up to approximately thirty-one (31) feet.  Conditions noted on or about 6/12/15.
