29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: a) On or about 6/29/2015, Bivi tech area: The working area around the shredder motor was not provided with a guardrail system or other fall protection. Employees were exposed to falls of six feet. b) On or about 6/29/2015, Wash Plant #2: The platform around the separating drum was not provided with a midrail, and had a top rail at a height of 34 inches from the walking surface. This exposed employees to falls of 20 feet. c) On or about 6/29/2015, Wash Plant #2: The platform over the Mag Tank was not provided with a midrail, and had a top rail at a height of 36 inches from the walking surface. This exposed employees to falls of 12 feet. d) On or about 6/29/2015, Wash Plant #2: The platform over the Mag tank had an opening in the floor of the platform that was 1.5 feet by 2.0 feet, and was not provided with a cover or guard rail system. This exposed employees to falls of 12 feet into the tank.
