29 CFR 1926.404(b)(1)(ii): All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which were not a part of the permanent wiring of the building or structure and which were in use by employees, did not have approved ground-fault circuit interrupters for personnel protection.  Receptacles on a two-wire, single-phase portable or vehicle-mounted generator rated not more than 5kW, where the circuit conductors of the generator were insulated from the generator frame and all other grounded surfaces, need not be protected with ground-fault circuit interrupters:    Near the east side exterior of the house, there was no assured equipment grounding conductor program or ground fault circuit interrupter protection for employees that used extension cords to power 110 VAC circular saws to cut lumber for formwork.      3 R, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.404(B)(1)(II) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1042175, CITATION NUMBER 2, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON MAY 15, 2015, WITH RESPECT TO A WORKPLACE LOCATED AT A NEW SINGLE FAMILY RESIDENCE PROJECT, BONITO STREET, DEDEDO, GUAM.
