29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25m) wide or less, was not protected from falling by guardrail systems, safety net systems or personal fall arrest systems:     (a) At 3813 Orangepoint Rd. in Valrico (FL), roofing employees without fall protection were patching a small area toward the north end section of a single story residential roof with a measurement from the edge to ground of approximately 9.5 feet, on or about 07/16/2015.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems or personal fall arrest systems:  (a) At 3813 Orangepoint Rd. in Valrico (FL), a roofing employee who was not using any means of fall protection while removing aluminum gutters at the south edge of the two story section of a split level residential roof approximately 15 feet from the edge of the roof to the ground, on or about 07/07/2015.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    a) At 3813 Orangepointe Rd. Valrico  (FL) where employees were working a full body harness with a 50 foot lifeline was provided to employees for means of fall protection while working on a single story roof that had a measurement of approximately 9.5 feet to the ground from the edge, as observed on or about July 16, 2015.

Valrico (FL) where employees were using a full body fall harness with a 50 foot harness which was connected to a anchor placed on the middle of the split roof while working on the single story low sloped roof which measured 9.5 feet from the edge of the roof to the ground, as observed on or about July 16, 2015.
