29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  a. on or about February 15, 2019, on a 4:12 pitch residential roof, , three employees who operated pneumatic nail guns were not protected with eye protection while exposed to eye injuries from flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on February 15, 2019, when two (2) employees are working on a residential roof with a 4:12 pitch, located at 1102 Alabama Avenue, Lynn Haven, FL 32444 without fall protection and are exposed to 14-foot 6-inch fall hazards.    b. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on February 15, 2019, when an employee is working on a residential roof with a 4:12 pitch, located at 1102 Alabama Avenue, Lynn Haven, FL 32444 without fall protection and are exposed to 11-foot 3-inch fall hazards.     Wayne Gonzalez Roofing Contractor, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1195873, citation number 1, item number 1 and was affirmed as a final order on February 8, 2017, with respect to a workplace located at 41 Sea Gull Drive, Ormond Beach, FL 32176.    Wayne Gonzalez Roofing Contractor, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1166130, citation number 2, item number 1 and was affirmed as a final order on September 8, 2016, with respect to a workplace located at 5525 Osprey Ridge Drive, Lithia, FL 33547.    Wayne Gonzalez Roofing Contractor, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1076113, citation number 1, item number 1 and was affirmed as a final order on October 29, 2015, with respect to a workplace located at 3813 Orange Pointe Road, Valrico, FL 33596.
