Employees entering the point of operation to unjam packaging material from the Doboy #1 machine in an energized condition were exposed to pinch points at the machines package sealing jaws.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a) Imagine Baking, Inc. on or about July 09, 2015 at the address of 1034 Hancock St. Sandusky, OH: The employer failed to ensure that employees who are exposed to energized rotating parts during service work on the DoBoy #1 were adequately trained in the skills required for the safe application, usage, methods, and means to isolate and control potential hazardous energy sources. The employees tasked with service work on the aforementioned machine were not specifically trained to lockout the machine to protect against amputation injuries prior to performing unjamming work near the point of rotation.

Employees tasked with packing duties at the output side are exposed to amputations hazards. Packing workers retrieving product for boxing are operating within close proximity of the sealing and cutting tool. (b) Imagine Baking, Inc. on or about July 13, 2015 at the address of 1034 Hancock St. Sandusky, OH: The employer failed to ensure the rotating sealing and cutting tool contained on the Doboy #2 was adequately guarded. Employees tasked with packing duties at the output side are exposed to amputations hazards. Packing workers retrieving product for boxing are operating within close proximity of the sealing and cutting tool.
