29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Imagine Baking, Inc. on or about March 26, 2025, at the address of 1034 Hancock St. Sandusky OH: The employer did not ensure that energy isolating procedures were implemented prior to the performance of cleaning and sanitation duties on the die cutter belt in the bake shop. Mixing team employees performing these tasks in an operational condition were exposed to incoming nip point(s) on the conveyor. The employer's established energy control procedure [required under 29 CFR 1910.147(d)] was not adequately utilized before the employee began the aforementioned work task on the machine.
