29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  Site: 51 Jay Street Brooklyn, NY  On or about 7/14/15  a) Frequent and regular inspections of the job site and equipment were not made by a competent person. The pipe scaffold was not fully planked, they used the scaffold frame as a ladder and they did not have a demolition survey.

Site: 51 Jay Street Brooklyn, NY  On or about 7/14/15  a) Employees worked from a tubular welded frame scaffold which was not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  Site: 51 Jay Street Brooklyn, NY  On or about 7/14/15  a) Employees worked from a tubular welded frame scaffold which did not have a means of safe access. Workers climbed the frames and cross braces as a means of access.

29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations.  Site: 51 Jay Street Brooklyn, NY  On or about 7/14/15  a) No demolition survey was done. Employees were removing a section of a brick wall were exposed to the possibility of unplanned wall collapse while doing demo work.
