29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times. Bunkhouse; on or about 16 July 2015 and times prior thereto; the employer did not ensure that emergency exit lighting was available. Two instances were noted were emergency exit lights located in the hallway areas were inoperative. This condition exposed employees to delays in exiting the building, smoke inhalation, fire, and tripping hazards.

29 CFR 1910.304(a)(2): Grounded conductors were attached to terminals or leads so as to reverse designated polarity. Wood shop; on or about 16 July 2015 and times prior thereto; a ceiling mounted Safeway electrical cord reel, model 945-3, serial number FR2048 was plugged into an energized electrical outlet that was wired with reverse polarity. This condition exposed employees to electrical shock and burn hazards.
