29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a. On or about July 16, 2015, front of the building, employees were using one side of a Louisville 16-foot ladder that was not able to be extended 3 feet past the landing and was not secured to prevent accidental displacement and a grabrail was not provided. b. 29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. On or about July 16, 2015, roof area, employees installing roofing shingles on a 7:12 pitch roof were not protected from falling up to 12 feet 4 inches by the use of a guardrail system, safety net system, personal fall arrest system or any other type of fall protection method. Duffield Aluminum Inc. dba Duffield Home Improvements was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 315478008, citation number 01, item number 002 and was affirmed as a final order on August 15, 2011, with respect to a workplace located at 3641 NW 13th Street, Gainesville, Florida 32609.
