On or about June 12, 2019, on the roof, two roofing employees were not provided eye protection when operating a pneumatic nail gun, exposing them to an eye injury hazard.   Duffield Aluminum, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1078605, Serious citation number 1, item number 2and was affirmed as a final order on September 14, 2015, with respect to a workplace located at 5542 NW 43rd St Gainesville, Florida.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.    On or about June 12, 2019, on the 6:12 pitch roof, three roofing employees installing shingles were exposed to a 9-feet fall hazard in that they were not protected by a fall protection system.  Duffield Aluminum, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1193835, Repeat serious citation number 1, item number 1 and was affirmed as a final order on December 21, 2017, with respect to a workplace located at 3147 SW 106 St, Gainesville, Florida.  Duffield Aluminum, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1078605, Repeat serious citation number 2, item number 1 and was affirmed as a final order on September 14, 2015, with respect to a workplace located at 5542 NW 43rd St Gainesville, Florida.

On or about June 12, 2019, the employer had not provided adequate fall protection training to employees for fastening anchor plates used as part of personal fall protection systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.    On or about June 12, 2019, above the side door entrance, three employees used the roof access ladder that was extended 33-inches of the required 36 inches above the roof eave edge.

On or about June 12, 2019, above the side door entrance, the roof access ladder was placed at 63-degrees, 12-degrees shallower than required by the standard.
