29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:  On or about July 16, 2015, on equipment such as, but not limited to, the MSL cleaner:  lockout steps used by technicians were not included in the machine specific procedures.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control:    a) On or about July 16, 2015, on equipment such as, but not limited to, the MSL beater machine:  The step to verify that the equipment was de-energized was not specific and said to ensure that energy sources(s) are 100% disconnected by operating the start button or other normal operating controls.      b) On or about July 16, 2015, on equipment such as, but not limited to, the Suessen Link Spinning machine:  The step to verify that the equipment was de-energized was not specific and said to ensure that energy sources(s) are 100% disconnected by operating the start button or other normal operating controls.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  On or about July 16, 2015, authorized employees had not been adequately trained to understand that affected employees could not assist in removing chokes.  Affected employees had not been adequately trained to understand that they could not assist in removing chokes.  Affected employees have not been trained to know the energy sources, how to lock out the machines, and do not have locks that would permit them to lock out machines.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
