29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:   On or about July 22, 2015, employees engaged in framing activities worked from a job made scaffold that was not designed by a qualified person.  Therefore, the scaffold construction lacked the required load capacity ratings, safety factors, and stability assurance.  Employees were exposed to the hazard of falls.   The JCO 1st Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(a)(6) which was contained in OSHA inspection number 314932310, citation number 1, item number 1 and was affirmed as a final order on June 24, 2011, with respect to a workplace located at 8421 N. 76th E. Ave., Owasso, OK 74055.  The JCO 1st Construction, Inc.,  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(a)(6), which was contained in OSHA inspection number 942227, citation number 1, item number 1 and was affirmed as a final order on December 27, 2013, with respect to a workplace located at 9109 N. 96th E Ave. Owasso, OK 74055.

Employees worked at heights greater than 6 feet above ground and lower level.  Employees were exposed to the hazard of fall.  The JCO 1st Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 942227, citation number 1, item number 2 and was affirmed as a final order on December 27, 2013, with respect to a workplace located at 9109 N 96th East Ave., Owasso, OK 74055. 29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:   On or about July 22, 2015, employees were not provided a ladder to access and egress the roof and upper level for framing/decking activities.  Two employees jumped from the nine (9) feet first level framing to the ground level.    The JCO 1st Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1051(a), which was contained in OSHA inspection number 942227, citation number 1, item number 4 and was affirmed as a final order on December 27, 2013, with respect to a workplace located at 9109 N 96th E. Ave., Owasso, OK 74055.
