29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:   On or about February 15, 2017, an employee engaged in residential siding work activities, worked from a job made scaffold that was not designed by a qualified person.  The scaffold construction and design lacked the required load capacity rating, safety factors, and stability assurance.  Employees were exposed to the hazard of falls.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide   On or about February 15, 2017, and at times prior thereto, the employer did not assure the working/walking surface of a job made surface was at least 18 inches wide.  The employee was exposed to the hazard of falls.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.     On or about February 15, 2017, and at times prior thereto, the employer did not ensure safe access was provided to an employee working from the second level of a job mad scaffold.  The employee was exposed to the hazard of falls.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  On or about February 15, 2017, and at times prior thereto, an employee performing siding work activities from a job made scaffold at a height greater than10 feet above ground level was not protected from falling.  The employee was exposed to the hazard of falls.
