29 CFR 1910.147(c)(7)(i)(C): All other employees whose work operations are or may be in an area where energy control procedures may be utilized, were not instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out:      On or about July 6, 2015 at worksite 4231 Clary Blvd, Kansas City, Missouri-employees were exposed to amputation, and caught by hazards when attempting to unclear a jam on press machine 24 on line 4 of the production line.    Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.147(c)(6)(ii)       The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.        On or about July 6, 2015 at worksite 4231 Clary Blvd, Kansas City, an examination of lockout/tagout program elements determined that periodic inspections of energy control procedures were not accomplished and certified.            Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1904.39(a)(2):     The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.       (a) On or about July 6, 2015 at worksite 4231 Clary Blvd Kansas City, Missouri. Aspen Products Inc., an employee's finger tip was amputated and the employer did not report the amputation to OSHA until July 16, 2015.      NO ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM.
