29 CFR 1926.453(b)(2)(v): The employer shall ensure that a body belt be worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about July 2, 2015, at a construction site in Houston, TX near 1330 E. Nasa Parkway, two employees were working from an aerial lift, Model # HB62, Serial# 77673, that extended a least 20 feet from the ground, without the use of personal fall protection.
