29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points: a. On or about May 30, 2017 and times prior thereto, on the NW side of the roof where two extension cords were routed through metal doors without being protected from damage. b. On or about May 30, 2017 and times prior thereto, on the SE side of the roof where two extension cords were routed through metal doors without being protected from damage.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about May 30, 2017, at or near 15 Greenway Plaza, employees were using a Bee Access Platform with the electrical cord to the hoist assembly not having strain relief.
