29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide: a) Jobsite, on or about July 23, 2015, employees used wooden planks that were resting on wooden uprights to elevate their height to perform framing and truss work. The wooden planks were secured and supported to form a supported scaffold system with wooden planks that were about 7 inches wide, exposing employees to a 5.5-foot fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) Jobsite, on or about July 23, 2015, employees were not provided with adequate access onto the wooden scaffold platform and roof and were required to step greater than 2 feet to access each level, exposing employees to fall hazards.
