29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  a) Jobsite, on or about July 23, 2015, employees used wooden planks that were resting on wooden uprights to elevate their height to perform framing and truss work.  The wooden planks were secured and supported to form a supported scaffold system with wooden planks that were about 7 inches wide, exposing employees to a 5.5-foot fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a) Jobsite, on or about July 23, 2015, employees were not provided with adequate access onto the wooden scaffold platform and roof and were required to step greater than 2 feet to access each level, exposing employees to fall hazards.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Jobsite, on or about July 23, 2015, while installing roof truss and performing other overhead work, employees were allowed to work below and nearby without head protection, exposing employees to struck-by hazards.    The Mejia Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100 (a), which was contained in OSHA inspection number 1038907, citation 1, item number 1 and was affirmed as final order May 19, 2015, with respect to workplace located at 130 Saratoga Drive, Rincon, GA 31326.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Jobsite, on or about July 23, 2015, employees were allowed to work performing framing activity such as, but not limited to installing roof truss without fall protection, exposing employees to a fall hazards up to 10 feet.     The Mejia Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b) (13), which was contained in OSHA inspection number 1038907, citation 1, item number 3 a and was affirmed as final order May 19, 2015, with respect to workplace located at 130 Saratoga Drive, Rincon, GA 31326.
