29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:     On or about July 28, 2015 at a worksite located at 3265 West Marker Street, Fairlawn, OH, during commercial construction activities, the employer did not ensure employees were properly protected from electrical hazards while working on a roof with a service entrance electrical line in close proximity to the work area.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    On or about July 28, 2015 at a worksite located at 3265 West Marker Street, Fairlawn, OH, during commercial low slope roofing activities the employer did not ensure employees were protected from fall hazards by ensuring a guardrail system, safety net system, personal fall arrest system, or a safety monitoring system was being used.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:     On or about July 28, 2015 at a worksite located at 3265 West Marker Street, Fairlawn, OH, during commercial construction activities, the employer did not ensure employees were provided a ladder for a change in elevation on the roof. Employees were accessing different areas of the roof by climbing a stack of insulation material.
