29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:     On or about July 28, 2015 at a worksite located at 3265 West Marker Street, Fairlawn, OH, during commercial construction activities, the employer did not ensure employees were properly protected from electrical hazards while working on a roof with a service entrance electrical line in close proximity to the work area.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    On or about July 28, 2015 at a worksite located at 3265 West Marker Street, Fairlawn, OH, during commercial low slope roofing activities the employer did not ensure employees were protected from fall hazards by ensuring a guardrail system, safety net system, personal fall arrest system, or a safety monitoring system was being used.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:     On or about July 28, 2015 at a worksite located at 3265 West Marker Street, Fairlawn, OH, during commercial construction activities, the employer did not ensure employees were provided a ladder for a change in elevation on the roof. Employees were accessing different areas of the roof by climbing a stack of insulation material.
