Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. a) Durable Rooftop Solutions, LLC on or about October 8, 2019, at the site location of 3516 Airport Highway Toledo, OH: The employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations. The employees did not recognize the hazards associated with working from an elevated location of 18 ft. without the benefit of fall protection.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder. a) Durable Rooftop Solutions, LLC on or about October 8, 2019, at the site location of 3516 Airport Highway Toledo, OH: The employer did not ensure the side rails of the 28 feet aluminum extension ladder, extended at least three feet above the landing surface. Six employees were exposed to falls of up to 18 feet while using the ladder to access the south side of the roof.

The employees were exposed to a potential 18 ft. fall to the ground below.
