29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    A. Durable Rooftop Solutions LLC worksite located at 429 Walton St., Willard, Ohio: On or about February 12, 2016, an employee fell thru an uncovered 50" x 40" HVAC roof opening approximately 17.50 feet to the floor below. The employee was installing a PVC membrane on the roof at the time of the incident. The employee sustained serious injuries related to the fall.     Durable Rooftop Solutions LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501, which was contained in OSHA inspection number 1080500, citation number 1, item number 2 and was affirmed as a final order on September 3, 2015 with respect to a workplace located at Fairlawn, Ohio.
