29 CFR 1910.95(c)(1):  A continuing, effective Hearing Conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8-hour Time-Weighted Average sound level (TWA) of 85 decibels (dBA):    a) ICS Corporation, Philadelphia, PA - BOWE folding machine room:  The employer did not provide a continuing, effective Hearing Conservation Program for employees exposed to noise levels above 85dBA.  An employee working in the BOWE folding machine room was exposed to continuous noise levels of 87.8dBA for an 8-hour TWA, which is equivalent to a dose of 74.09%.  This exposure exceeds the Action Level of 85 dBA.  The employee was sampled for 405 minutes.  Zero exposure was assumed for all unsampled time periods.    Violation occurred on/or about August 6, 2015.
