29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a.) Ball Bounce and Sport, Inc. dba Hedstrom Plastics located at 1860 S. Baney Rd. Ashland, OH; On or about July 22, 2015, the employer did not ensure machine specific energy control procedures were developed, documented and utilized for the Roto #1 molding machine. On July 22, 2015, an employee was attempting to align bolt holes while performing a mold change and received a partial finger amputation in that the equipment was not deenergized. b.) Ball Bounce and Sport, Inc. dba Hedstrom Plastics located at 1860 S. Baney Rd. Ashland, OH; On or about July 29, 2015, the employer did not ensure machine specific energy control procedures were developed, documented and utilized for the Roto #1 molding machine. Employees are required to service molds and do not deenergize the machine, exposing them to struck by hazards.

Ball Bounce and Sport, Inc. dba Hedstrom Plastics located at 1860 S. Baney Rd. Ashland, OH; On or about July 29, 2015, the employer did not ensure employees were trained in the purpose and function of the energy control program and understood by employees operating the Roto #1 rotational molding machine. Employees are required to service and change out molds and are exposed to struck by hazards from unexpected start up of the equipment.
