On July 22, 2015, an employee was attempting to align bolt holes while performing a mold change and received a partial finger amputation in that the equipment was not deenergized.          b.)    Ball Bounce and Sport, Inc. dba Hedstrom Plastics located at 1860 S. Baney Rd. Ashland, OH; On or about July 29, 2015, the employer did not ensure machine specific energy control procedures were developed, documented and utilized for the Roto #1 molding machine. Employees are required to service molds and do not deenergize the machine, exposing them to struck by hazards.
