29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling:   a. On or about January 29, 2019, employees were exposed to fall hazards of approximately 5.5 feet when accessing the elevated work platform at Machine #2 to load/unload molds.   b.  On or about January 29, 2019, employees were exposed to fall hazards of approximately 7 feet when accessing the elevated work platform at Machine #3 to load/unload molds.    c.  On or about March 5, 2019, employees were exposed to fall hazards of approximately 6 feet when accessing the elevated work platform at Machine #4 to load/unload molds.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. On or about March 5, 2019, the employer did not ensure that all employees who worked in the hazard area of rotocast machine #1 were protected from struck-by injuries created by the machine's rotating arms and molds.  Ball, Bounce and Sport, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1081158, citation number 1, item number 3 and was affirmed as a final order on October 22, 2015, with respect to a workplace located at 1860 Baney Rd., Ashland, Ohio 44805.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a. On or about January 25, 2019, the employer did not develop and document energy control procedures for machines such as, but not limited to, the rotocast machines #2, #3, and #4.  Employees that clean the machines were exposed to struck-by hazards.  Ball, Bounce and Sport, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1081158, citation number 1, item number 1a, and was affirmed as a final order on October 22, 2015, with respect to a workplace located at 1860 Baney Rd., Ashland, Ohio 44805.

Test voltages and/or the maximum intervals between tests were not conducted in accordance with Table I-4 and Table I-5.  a. On or about February 5, 2019, electrical protective rubber gloves, used by maintenance during electrical troubleshooting and to verify absence of energy, were not tested at least once every six months.
