Subcon Tool Accutool Machine Group Inc. dba Accutool; warehouse - On or about 7/29/15 and at time prior thereto, an employee was operating a Clark industrial fork truck that was not equipped with a seat-belt.

Subcon Tool Accutool Machine Gropu Inc. dba Accutool; warehouse - On or about 7/29/15, the company had a Clark forklift that had a damaged/modified operators seat that was modified with cardboard. A 'replacement seat" was fashioned out of cardboard and used on the industrial truck.
