29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:      A.)  Subcon Tool Accutool Machine Group Inc. dba Accutool - On or about 7/29/15, the company had not performed an annual periodic inspection of the lockout/tagout procedures with all authorized employees.  The last periodic inspection of the procedures was in 2005.  Employees performed servicing and maintenance on machines that included, but were not limited to, CNC's and OMAX waterjet.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      A.)  Subcon Tool Accutool Machine Group Inc. dba Accutool; warehouse - On or about 7/29/15 and at time prior thereto, employees were operating an Amada horizontal band saw without a guard over the unused portion of the blade.    B.)  Subcon Tool Accutool Machine Group Inc. dba Accutool; warehouse - On or about 7/29/15 and at time prior thereto, employees were operating a Kalamazoo horizontal band saw without a guard over the unused portion of the blade.
