29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:    29 CFR 1910.147(c)(4)(ii)(A): A specific statement of the intended use of the procedure;    29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;    29 CFR 1910.147(c)(4)(ii)(C): Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and    29 CFR 1910.147(c)(4)(ii)(D): Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.    a. At the workplace, the employer did not provide energy control procedures that included a specific statement of intended use, specific sequential procedures for securing the machine, and specific requirements to verify the effectiveness of the energy control measures, for employees that were engaged in activities on equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a.  At the workplace, an Edgewater quilting machine, model 6413, serial # M80396, had a needle bar that was not guarded to prevent the operator from accidental contact with moving parts.    b.  At the workplace, an Edgewater quilting machine, model 8413, serial # M79314, had a needle bar that was not guarded to prevent the operator from accidental contact with moving parts.    c.  At the workplace, a Skil Drill Press, model number unknown, had a rotating chuck that was not guarded exposing employees to a caught-in hazard.    d. At the workplace, a Rigid Drill Press, model DP15000, had a rotating chuck that was not guarded exposing employees to a caught-in hazard.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures.  a.  At the workplace, in the North building, there was an electrical box mounted on a Vertrod machine that did not have a cover installed to prevent accidental employee exposure to contact with electrical current from exposed open wiring.

29 CFR 1910.334(b)(2): Reclosing circuits after protective device operation. After a circuit is deenergized by a circuit protective device, the circuit may not be manually reenergized until it has been determined that the equipment and circuit can be safely energized. The repetitive manual reclosing of circuit breakers or reenergizing circuits through replaced fuses is prohibited.    a.  At the workplace, in the North building, employees repetitively replaced fuses to reenergize the Vertrod machine electrical circuit that was deenergized by the circuit protective device, thereby exposing employees to electrical hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a.  At the workplace, in the Fiberglass Department, a PA 7000 Scheer saw did not have the point of operation adequately guarded, thereby exposing employees to an amputation hazard.  On July 22, 2015, an employee suffered multiple finger amputations on both hands while operating a PA 7000 Scheer saw.    b.  At the workplace, in the North building, a grommet machine equipped with a two hand control device had one of the push buttons taped in the pushed position, therefore the point of operation was not adequately guarded.     Kinetics Noise Control was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 111186, citation number 1, item number 1 and was affirmed as a final order on April 12, 2012, with respect to a workplace located at 6300 Irelan Place, Dublin, Ohio 43017.
