29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard. On or about June 30, 2015, when changeovers were required on the QB1 Thermoforming Press, an employee changing cylinders on top of the QB1 was standing at a height of over six feet above the floor without using fall protection.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and other moving parts: On or about July 30, 2015, employees were exposed to: a) The pinch point between the backward and forward motion of the shuttle table and the front frame of the conveyor of the QB1 infeed system. b) The hazard of the rotating upper and lower shafts of the infeed system of the QB1.
