On or about February 17, 2022, the employer did not ensure that employees using the SkyJack scissors lift (S/N:27009390) were properly trained per the requirements of 29 CFR 1926.454, which is found in 29 CFR part 1926, subpart L (Scaffolds).

29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. On or about February 17, 2022, procedures were not developed for controlling hazardous energy for the Garage door Motor in the Extrusion Resin Dock Door #12, exposing employees to hazards including, but not limited to, point of operation, ingoing nip points, and rotating parts.
