29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. a) Excavation located at 2117 Jane Lyle Rd. SE, Smyrna, GA 30080: Employees were not protected from eye injuries while cutting a steel pipe with a Stihl 14" cutoff saw (Model TS420) as this tool was operated without eye protection. Hazardous conditions observed by CSHOs on 8/4/15.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that the excavation was sloped at an angle steeper than one horizontal to one vertical. a) Excavation located at 2117 Jane Lyle Rd. SE, Smyrna, GA; The employer allowed his employee to work in an excavation/trench that was 20' long by 6' wide by 6' deep with no protective system, i.e. trench box, shoring and/or sloping in place to prevent collapse. Hazardous conditions observed by CSHOs on 8/4/15.
