29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level: On or about July 9, 2015, at Lot 93A, 3534 Hideaway Lane, Loganville, Georgia: An employee performing siding work was exposed to a fall hazard of approximately 17.2-feet when he used a 40-foot Louisville aluminum extension ladder at an approximately 110 degree angle and was not used on a level surface or secured to prevent displacement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about July 9, 2015, at Lot 93A, 3534 Hideaway Lane, Loganville, Georgia: a) One employee performing siding work was exposed to a fall hazard of approximately 17.2-feet when he worked from an approximately 10/12 slope gables roof without using fall protection. b) One employee performing siding work was exposed to a fall hazard of approximately 16-feet and greater when he worked from an approximately 10/12 slope garage roof without using fall protection. Adolfo Garcia-Salazar was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 988966, citation number 1, item number 3, and was affirmed as a final order on November 24, 2014, with respect to a workplace located at 4977 Ansbury Place, Acworth, Georgia.
