29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:    A.)  Kold-Draft International, LLC, 1525 East Lake Road, Erie, PA  16511, on or about August 4, 2015 and times prior thereto:  Employees did not have initial employee fire extinguisher training.  Fire extinguishers were deployed in the workplace.  The fire extinguishers included but were not limited to multi-purpose dry chemical and water mist extinguishers.    A.)  Kold-Draft International, LLC, 1525 East Lake Road, Erie, PA  16511, on or about August 4, 2015 and times prior thereto:  Employees did not have annual fire extinguisher training.  The last fire extinguisher training was held on April 29, 2013. Fire extinguishers were deployed in the workplace.  The fire extinguishers included but were not limited to multi-purpose dry chemical and water mist extinguishers.

The electrical knockout box was on the table and not mounted to a surface as designed by their listing and labeling.    B.)  Kold-Draft International, LLC, 1525 East Lake Road, Erie, PA  16511, bin area, on or about August 25, 2015 and times prior thereto:  Employees were utilizing a cord and plug, two outlet electrical knockout box to power equipment including, but not limited to, a Samsung cell phone charger.  The electrical knockout box was on the table and not mounted to a surface as designed by their listing and labeling.
