29 CFR 1910.212(a)(3)(ii): The point of operation machine guards were not designed or constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.   Machine Shop Area, Boschert Notching Shear (Type L.B.-14, Model NR-52).  On or about July 28, 2015, employees operated the Boschert Notching Shear to cut corners and trim the ends of metal pieces.  Employees were exposed to amputation and laceration hazards in that the point of operation on the machine was not adequately guarded:   a) The guard for the two 9" v-shaped cutting blades on the shear machine did not prevent operators' fingers from entering the point of operation.        b)  Operators used a foot pedal control to activate the downward shearing action of the cutting blades.  The foot pedal control was not in a fixed position at a safe distance from the point of operation to protect the operator, nor was it guarded to prevent indadvertent activation.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
