29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: For the facility at 5205 Adamo Dr., Tampa, FL 33604, as observed on or about 08/05/2015, electrical equipment was used in an unsafe manner exposing employees to potential fire hazards or electric shock hazards in that: a) Portable power taps were used on the South side of the building to supply power to microwaves and portable fans. The power taps were used in an industrial environment, exposed to rain water from roof leaks, showed signs of burn marks on them, and one of the outlets had a ground prong broken off in it. b) Portable fans used on the south side of the building, were exposed to rain water from roof leaks, and were siting in puddles of water.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) For the facility at 5205 Adamo Dr., Tampa, FL 33604, as observed on or about 08/05/2015, electrical equipment was used in an unsafe manner exposing employees to potential fire hazards or electric shock hazards in that extension cords were used with their outer insulation insulation broken away from their plug which creates a strain on the inner conductors.
