Trillium Farms Holdings, LLC located in Harpster, Ohio: On or about November 23, 2015, the employer did not implement the procedures and practices necessary for safe entry into permit required confined spaces. Employees routinely entering the egg storage silos to perform cleaning in place (CIP) operations were not filling out confined space entry permits, performing air testing, and locking out the agitator blades prior to entry.
