29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a.) On or about March 3, 2017, the employer did not ensure employees were protected while standing on the top of the truck cab in the load out trucking area. Employees are required to clear out the egg shell chute and stand on the truck cab to reach the chute and are exposed to falls of approximately 9 feet to the concrete below.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a.) On or about March 3, 2017, maintenance employees were exposed to amputation injuries in that the egg shell auger in the Truck Loading Area was not locked out prior to cleaning. The employer did not ensure procedures were developed to control the electrical energy source prior to the maintenance tasks.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: Although the employer trained employees in the purpose and function of the energy control program, on or about March 8, 2017, the employer did not assure that an energy control program for employees cleaning the egg shell auger in the shell bay completely controlled the electrical energy source, exposing them to amputation hazards from the auger.
